    – TIN (Tax Identification Number)
    – Company telephone number
    – Company address
    – Type of business

  3. Payment Information
    – For bank transfers, please request US bank details from your Program Advisor.
    – All payments will be made directly to Maxwell Leadership USA.
    – For credit card payments, please proceed in choosing your payment options below. Your payment is made directly to Maxwell Leadership USA.
